As our patient you will have access to our secure Patient Portal. This allows you to communicate with us electronically to ask non-urgent medical questions, request prescription refills, request or change appointments, and reach our surgery scheduling and billing departments. You can also enter or update your demographic and medical information in your chart and review your test results. We are always available by phone for matters requiring immediate attention.

You may fill out the pre-registration form found on the PATIENT PORTAL link to the right, email us with your phone number, or call us by phone.  In all cases we will speak with you to confirm your identity and then issue your User ID.

It is helpful to have your prior medical records sent to us prior to your arrival for your visit. Please download the record request form from the link at the right, fill it out and fax it to your recent physician's office.
